if it is like my old 50's truck there is a square head bolt that removes in the same way the larger outer nut does, otherwise tightening the outer nut would loosen the inner nut, there is a special socket that fits this square bolt-do not try to use a big adjustable wrench it will round off the nut-experience!!

The inner fasteners on old dual wheel trucks are, more often than not, well rusted in place and tough to remove. It will likely require a square socket and a strong impact air wrench to break it loose. If you don't have access to those, I suggest that you take it to a tire shop to have it removed before you damage the studs. Some anti seize compound on the threads will help you the next time you need to pull the wheel(s).
